On-Line Registration | Please use this method if at all possible. This is the most efficient - saves your paper and postage AND data entry time for the Registrar. Be sure to click on SUBMIT at the end. If you are not returned to the Guild's Web site and an acknowledgement page, the registration was NOT submitted. |

Insurance Waiver You will need only one copy of the signed Insurance Waiver, but you must print and complete a label and receipt for each entry. |
Word Template | Open this file on-line and print a copy of the label and receipt for each entry. If you have MS Word, you can save the file to your computer for your records and fill in the information for each entry using your computer before printing. |

Boutique Consignment You will need a signed Insurance Waiver with your Boutique Inventory form. The waiver is the second page of either document. |
